  1. Application Letter.

  2. Applicant's ID card/passport.

  3. Articles of Association or the latest Amendment Deed for business entity applicants.

  4. Decree of Approval from the Ministry of Law and Human Rights (A.H.U) for the Articles of Association or the latest Amendment Deed for business entity applicants.

  5. Taxpayer Identification Number (NPWP).

  6. Business Identification Number (NIB).

  7. Environmental permit UKL-UPL / SPPL.

  8. Location permit.

  9. Statement of compliance with applicable regulations.

  10. Declaration of Truth and Validity of Documents.

  11. Company organizational structure.

  12. Reasons for employing foreign workers (TKA).

  13. KITAS (for RPTKA extensions).

  14. Completed RTKA form.

  15. Company domicile certificate from the local government.

  16. Valid mandatory labor report (as per Law No. 7 of 1981).

  17. ID card and the accompanying worker listed in the Mandatory Labor Report.

  18. Report on the implementation of training and education for technology and expertise transfer, including training certificates.

  19. Employment contract.

  20. Valid RPTKA approval (for RPTKA extensions).

  21. Recommendation for the position held by the foreign worker from a technical institution for specific positions and business fields.

Notif IMTA

Notification of Foreign Worker Work Permit | 外籍勞工工作許可證通知
  1. Valid RPTKA.

  2. Valid KITAS (for IMTA extensions).

  3. Foreign worker's (TKA) photograph, 3x4 size (red background).

  4. Notification Application Letter.

  5. Foreign worker's (TKA) diploma (with translation).

  6. Reference letter/Work experience of the foreign worker (TKA).

  7. Curriculum Vitae (CV).

  8. Insurance policy.

  9. Taxpayer Identification Number (NPWP) for the foreign worker (TKA) (for IMTA Notification Extension).

  10. BPJS Employment (for IMTA Notification Extension).

  11. BPJS Health (for IMTA Notification Extension).

  12. Employment agreement for the contract period as per RPTKA between the foreign worker (TKA) and the employer.

  13. ID card and accompanying worker listed in the Mandatory Labor Report.

  14. Supporting documents for the work location.

  15. VITAS Application Letter.

  16. Candidate foreign worker's (TKA) passport.

  17. Declaration and Guarantee Letter from the employer.

  18. Company’s bank statement for the last 3 months with a minimum balance equivalent to $1,500.

Mandatory Employment Report | 強制性就業報告
  1. Form completion.

  2. Articles of Association of the company.

  3. Approval of the Articles of Association of the company.

  4. Company's Business Identification Number (NIB).

  6. Environmental permit UKL-UPL / SPPL.

  7. Location permit.

  8. BPJS membership.

  9. ID card and photograph of employees under the Fixed-Term Employment Agreement (PKWT).

  10. Number of workforce requirements.
